Work Activity Profiles (WAPs) are forms that you, the trainee, fill out every week. You list the “on-job” activities you have completed that week on your form. Each activity relates to one unit standard. The unit standard number is written beside the activity.

WAPs are included in the training manual you get when you sign a training agreement with your employer and ESITO. They show your training experience. It is very important that you complete them accurately each week.

Follow these guidelines to make sure the WAP works for you:

• Photocopy blank copies of your Work Activity Profiles and keep them so you can complete one a week.
• Don’t borrow other peoples’ Work Activity Profiles sheets.  They may not be the same.
• Make sure you keep copies of your completed Work Activity Profiles for your own file.
• Send your Work Activity Profiles for the previous week to ESITO by 4pm, the following Monday, either by fax to 07 834 8160 or post to ESITO, PO Box 1245, HAMILTON 3240.
